Weakley County Rehabilitation & Nursing Center
Since our origin in the mid-1800s as the Weakley County Poor Farm, Weakley County Rehabilitation & Nursing Center has stood the test of time as a constant quality healthcare provider serving the people of Northwest Tennessee. Weakley County Rehabilitation & Nursing Center transitioned from a Poor Farm to become a CMS licensed nursing home in 1958. The nursing home is a non-profit and owned by the people of Weakley County.
Our goal at Weakley County Rehabilitation & Nursing Center is to be a positive partner to the elderly population of Weakley County. We provide a full range of rehabilitation services including Physical, Occupational and Speech Therapy to the residents of Weakley and surrounding Counties. We also offer these services in an outpatient setting allowing us to be a supportive partner in your loved one’s healthcare journey as they age in our community.
In some cases our healthcare facility becomes home to residents. This is when we become a community, not just a healthcare provider. The whole life of our resident is important. At Weakley County Rehabilitation & Nursing Center we understand the mental, physical and spiritual needs of our residents and families. Our long term care services are resident – centered and promotes independence for our resident and their families while maintaining dignity.
